Acuity accepts patient referrals from a variety of sources: family physicians, specialists, case managers, social workers, and nursing homes. The patient or even a family member can request a clinical consultation. Because patients requiring our services are acutely ill, referrals are accepted day and night.  Once a referral has been completed, a clinical liaison will visit you in the hospital, explain the transfer process and answer any questions that you may have.
